Instructions for completing your application:
- Download the application by clicking on this APPLICATION LINK.
- Open the application in Adobe Reader.
- Save the application with your name by clicking on File > Save As. (For example: USI_Alumni_Application_Fall_2022_Neil_Armstrong)
- Complete your application (save your application occasionally so as not to lose any of your answers).
- When you are finished completing your application please print it and submit it with the other requested documents in ONE ENVELOPE via mail postmarked on or before July 5 2024 to:
